Rural communities often face unique challenges in connecting patients with the full range of health and social services they need. Missouri’s Transformation of Rural Community Health (ToRCH) program is helping address these challenges by creating stronger networks between rural hospitals, healthcare providers, and community-based organizations. This webinar will explore how the ToRCH pilot leveraged Unite Us’ closed-loop referral platform to support coordinated care, connect patients to resources, improve outcomes, and reduce barriers to care.
Participants will gain insight into lessons learned from the ToRCH pilot, the role of rural hospitals as community health hubs, and how ToRCH Care is expanding these efforts as part of Missouri’s Rural Health Transformation Program. Attendees will also learn how collaboration and technology can help rural stakeholders prepare for future opportunities to strengthen care delivery across their communities.
